These Terms and Conditions of Website Use (“Terms”) govern your access to and use of the website operated by Claris Mental Health LLC (“Claris”, “Claris Mental Health”, “we,” “us,” or “our”), including the content, features, forms, links, and other resources available through the website (collectively, the “Website”). Please read these Terms carefully. By accessing, browsing, or using the Website, you agree to these Terms and our Website Privacy Policy. If you do not agree with these Terms, please do not use the Website.

These Terms govern use of the Website. They do not replace any consent for treatment, financial agreement, telehealth consent, Notice of Privacy Practices, medication agreement, or other agreement that may apply if you become a patient of Claris Mental Health.

1. Website for Informational Purposes

The information provided through this Website is intended for general informational and educational purposes only.

Website content is not intended to constitute medical, psychiatric, psychological, legal, or other professional advice and should not be used as a substitute for evaluation, diagnosis, or treatment by a qualified healthcare professional.

Information concerning mental-health conditions, medications, treatments, or services is general in nature and may not be appropriate for a particular individual.

Do not disregard professional medical advice or delay seeking professional care because of information you have read on this Website.

2. No Provider-Patient Relationship

Accessing or using this Website does not establish a provider-patient relationship between you and Claris or any Claris healthcare professional.

Submitting an inquiry, appointment request, contact form, or other communication through the Website likewise does not establish a provider-patient relationship and does not guarantee that Claris will accept you as a patient.

A provider-patient relationship is established only after Claris has agreed to provide care and the applicable intake, consent, and other required processes have been completed.

3. Appointment and General Inquiries

The Website may permit you to request information or request an appointment.

Submission of an appointment request does not constitute confirmation of an appointment. Appointments are confirmed only after you receive confirmation from Claris or through an authorized scheduling system.

Please do not use general Website inquiry forms to provide detailed medical histories, medication information, or other sensitive clinical information unless specifically requested through an authorized secure system.

Clinical information may be collected through Claris’s authorized patient systems as appropriate.

4. Emergencies and Urgent Communications

This Website and its contact and appointment-request forms are not emergency services and are not continuously monitored. Do not use the Website, email, text messaging, or general inquiry forms to seek emergency medical or psychiatric help.

If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency department. If you are in crisis or having thoughts of suicide, call or text 988 to reach the 988 Suicide & Crisis Lifeline, available 24 hours a day, 7 days a week.

If you are a current Claris patient with an urgent concern that is not an emergency, call our office at 971-365-4513 or send a message through the patient portal. We aim to respond to messages within two business days, during regular business hours. Messages received after hours, on weekends, or on holidays will be answered the next business day.

Website communications should not be relied upon for urgent or time-sensitive clinical needs.

5. Clinical Services

Descriptions of conditions, treatments, medications, therapeutic approaches, or other services available through the Website are provided for general informational purposes.

Nothing on the Website guarantees that a particular treatment, medication, diagnosis, service, or clinical outcome will be appropriate or available for any particular individual.

Treatment recommendations are made only after appropriate clinical evaluation and are based on the professional judgment of the treating clinician, applicable standards of care, patient circumstances, and other relevant considerations.

Claris does not guarantee any particular treatment outcome.

6. Website Privacy and Health Information

Your use of the Website is also subject to the Claris Mental Health Website Privacy Policy.

If you become a patient, Claris’s collection, use, and disclosure of protected health information is also governed by its Notice of Privacy Practices and applicable federal and state law.

Communications transmitted through the internet, email, text messaging, or other electronic methods may involve privacy and security risks.

Claris uses administrative, technical, and physical safeguards as appropriate to protect information within its control, but no electronic communication or information system can be guaranteed to be completely secure.

Patients should use designated secure patient systems for clinical communications when instructed to do so.

8. Text and SMS Communications

If you provide a mobile phone number and agree to receive text messages from Claris Mental Health, you may receive service-related messages such as appointment reminders, scheduling updates, administrative follow-up, and billing notices.

Agreeing to receive text messages is not a condition of receiving care from Claris.

Message frequency varies. Message and data rates may apply.

You can opt out at any time by replying STOP. For help, reply HELP or contact us at 971-365-4513 or hello@clarismentalhealth.com

The Website may contain links to or integrate with websites, platforms, applications, scheduling systems, patient portals, payment processors, or other services operated by third parties.

Claris does not control third-party websites or services and is not responsible for their content, availability, security, privacy practices, or terms.

The inclusion of a link or integration does not constitute Claris’s endorsement of the third party or its products or services.

Your use of a third-party service may be governed by that third party’s own terms and privacy practices.

10. Permitted Use

You may use the Website only for lawful purposes and in accordance with these Terms.

12. User Communications and Feedback

If you voluntarily provide general feedback, suggestions, or ideas regarding the Website or Claris’s nonclinical services, Claris may use that feedback to improve its operations and services, subject to applicable privacy and healthcare laws.

You remain responsible for content you voluntarily submit and for ensuring that you have the right to submit it.

Nothing in these terms is intended to limit or exclude liability that cannot lawfully be limited or excluded.

These limitations relate to use of the Website and are not intended to waive or alter legal duties applicable to healthcare actually provided by Claris or its clinicians.

16. No Guarantee of Insurance Coverage or Benefits

Any information provided through the Website concerning insurance participation, benefits, reimbursement, or anticipated costs is general information and does not guarantee payment or coverage by an insurer.

Insurance networks, eligibility, benefits, deductibles, copayments, coinsurance, authorization requirements, and coverage determinations may change and are ultimately determined by the applicable health plan.

17. Reviews and Testimonials

Any patient reviews or testimonials that may appear on the Website or other Claris-controlled materials reflect the experiences and opinions of the individuals providing them.
